• Holodiastolic flow reversal noted in the distal transverse and descending aorta in patient with a bicuspid aortic valve with severe aortic regurgitation.
  • Spectral Doppler pattern in the descending aorta revealing holodiastolic flow reversal (flow above baseline).

The suprasternal notch view is a good view to obtain a peak gradient across a bicuspid aortic valve with stenosis (gradient can be obtained in the ascending aorta).  In addition, flow patterns in the descending aorta can be helpful to assess severity of aortic regurgitation (diastolic flow reversal can be noted in patients with moderate to severe aortic regurgitation).  Given the association of bicuspid aortic valve with coarctation of the aorta, this is an important view to assess the size of the aortic arch and for evidence of coarctation.
