• Tetralogy of Fallot
    • Moderately hypoplastic pulmonary valve
    • Turbulent flow across the pulmonary valve consistent with pulmonary valve stenosis 
    • Moderate sized patent ductus arteriosus with left to right shunting
  • Tetralogy of Fallot
    • Mildly hypoplastic pulmonary valve
    • Turbulent flow across the pulmonary valve consistent with pulmonary valve stenosis 
  • Spectral Doppler across the right ventricular outflow tract (RVOT) and pulmonary valve
    • D= Dagger-like Doppler pattern consistent with dynamic obstruction across the RVOT seconcdary to infundibular narowing
    • PV= Second superimposed Doppler pattern consistent with fixed obstruction across the pulmonary valve. 

Echocardiographic Assessment: Parasternal long axis (pulmonary valve)

  • Assess infundibular obstruction (2D, color and spectral Doppler, spectral Doppler both pulse and continuous)
    • Continuous wave Doppler interrogation of the right ventricular outflow tract demonstrates high-veloctiy flow with late-peaking, dagger-shaped flow consistent with dynamic infundibular obstruction. 
  • Assess pulmonary valve size and anatomy (2D, color, spectral Doppler both pulse and continuous)
  • Assess main pulmonary artery size branch pulmonary artery size and degree of the obstruction (2D, color and spectral Doppler both pulse and continuous)
